Clear and supportive documentation cannot be overemphasized in ophthalmology, just as it is requisite for all medicine. Let’s face it: payers are scrutinizing chart documentation more. Why? In theory, to find supportive documentation to pay the claim. In many instances, they find conflicting or missing information instead. Ensuring every patient encounter is accurately documented—with no conflicting information—makes a significant difference in patient care, billing accuracy, and compliance.
